In 1999 the U.S. military Joint Software System Safety Committee published a Software System Safety Handbook: A Technical & Managerial Team Approach. Of interest to software reuse and safety is Appendix D on COTS and NDI (Non-Developmental Items).

A particularly active group in software safety is the Automotive-SPIN initiative in Italy. Here is a presentation made by Intecs SpA on safety issues in embedded automotive software. The presentation covers a number of issues touched on in other areas of this site, such as the relationship between safety and reliability, the concept of safety integrity levels, relevant standards, and the like. It also presents a representative software safety architecture.
